Terminal Distributor License Required for Oxygen

We have recently been informed by OHCA Champion Partner Institutional Care Pharmacy (ICP) that a license as a Terminal Distributor of Dangerous Drugs is required by the Ohio Board of Pharmacy (OBP) for any facility in the possession of oxygen or medical gases.

Form #0600 – Application for Terminal Distributor (TDDD) License must be completed and returned to the Ohio Board of Pharmacy. Limited Category II states: this licensee may only possess, have custody or control of, and distribute only the dangerous drugs carrying the Federal Legend which are NOT controlled substances. This category includes medical gases.

The Limited License Personnel list must also be completed and returned with the license application. This list should include any personnel that have access to oxygen and medical gases.
