Description Duties and responsibilities MDS Participates in the MDS process and ensures completion of MDS sections to ensure that MDS’s are completed per OBRA guidelines and Medicare PPS guidelines. Attend weekly U.R. meetings Contributes to the accuracy of MDS assessments per federal and state regulations Monitors facility Case Mix and Quality Measures Promotes and supports resident choice through the MDS process Care Plans Complete comprehensive care plan per guidelines Performs Quarterly, Annual and Significant Change Assessments or Evaluations that include Pain Evaluation, Physical Restraints (including Decision Trees), Elopement, Braden Scale for Pressure, Bed Rail Evaluation, Psychoactive Medication Quarterly Evaluation (when appropriate), Fall Risk. Inform charge nurses of need to complete assessments or evaluations and obtain consents as necessary. Update both STNA and resident Care Plan chart as necessary Participates in Care Conference of new admits and long stay residents to ensure that Care Plan is reflective of current therapy recommendations and resident choice and goals as needed Levels of Care Complete Level of Care as needed Ancillary Responsibilities Gathering information from rounding on the units, reporting at Stand-up meeting and participating in follow-up measures determined after meeting Participates in Fall Committee Meeting and performance improvement Actively participates in Q.A.P.I. process · Attends training, competencies and in-service programs as required. Promotes and supports resident choice Assures resident safety Qualifications: · Must be a registered nurse (RN) having completed a minimum of the Essential MDS 3.0 Course. - Must have previous MDS experience...Preferably recent experience · Experience in everyday operations of the nursing facility · Strong organizational skills · Ability to motivate, multitask, and manage individuals and groups · Experience and understanding of basic computer skills (preferred) Physical requirements: · Bending, lifting, grasping, fine hand coordination, · Ability to distinguish smells, tastes and temperatures · Must have the ability to communicate accurately in writing and verbally to others · Ability to hear and respond to call system, alarms, and telephone
Salary Range $60,000 to $72,000 per year
