Requirements of Participation – Phase 2: Care Planning and Discharge Planning

Level: Intermediate

The new nursing home requirements of participation were published in October 2016 and will be implemented in three phases. This informative session will present the regulatory requirement changes for care plans and discharge planning effective in Phase 2 of regulatory implementation. The new regulations for Baseline Care Plans will be discussed, including strategies for rapid development of an acceptable care plan and inclusion of nursing assistants in the care planning process. Discharge planning requirements will be reviewed, including discharge planning considerations and sharing quality information about discharge destinations with residents and families. Participants will gain an understanding of how to prepare residents for successful discharge in the new regulatory environment.
